After placing your order, you should message us with the following information:
 - Medical Condition or Allergy (Printed on the front and the rear)
 - Name as you wish it to appear on the card
 - Date of Birth
 - Address
 - Medication details
 - 2 x Emergency Contact names and telephone number.
All fields are optional but recommended. 
Any fields not provided will be left blank. 
The PVC cards are not designed to be hand-written on, but can be with a permanent sharpie.
WE WILL COPY AND PASTE THE PROVIDED INFORMATION ONTO THE CARD FOR PRINTING.
PLEASE ENSURE ALL INFORMATION IS CORRECT BEFORE SENDING IT TO US.
